Account Linking

A lot of server have this feature that you can type /link in chat to get an custom code which you can use to connect you're Minecraft account with the Discord account at the server of them. Prob. their own discord bot too. Also they made it so you can get an Discord Rank Ingame automatic. Anyone knows how I can make that or any existing skrips/plugins?
