Flat World

To make your default world a flat world, go to your Dashboard > Settings > Level Type > Flat then you have to reset your world for it to take effect!

 

Creative

To make your default gamemode creative, type /defaultgamemode creative. You can do this from the server commands on the dashboard, or you can type it in game if you have "OP" or all permissions.

Another way you can create a flat world is with a plugin called Multiverse. Just run the following command in game. /mv create [world name] NORMAL -t FLAT. Then do /worlds to see a list of your worlds and /world [world name] or /mvtp [world name] to teleport to the world you created. Then to set the gamemode of that world to creative, teleport to the world and do /mvm set mode creative and the gamemode of the world should be create. Not this is all done with a plugin called Multiverse. If you don't have that, do it with the method Untreated said.
