No, you can use permission plugins like PermissionsEx, Luckperms, etc to grant certain perms to the player using ranks, I recommend watching a tutorial on how to use them (Luckperms is more user friendly)

The world should update correctly. Make a backup in case something goes wrong. You will need to explore new chunks to see new caves and stuff. You might get new terrain below the bedrock in already generated chunks depending on certain conditions. Nothing should interfere with user-generated structures.
